BSW ICB launches anticipatory care digital toolkit

A new toolkit has been launched by BSW Integrated Care Board to help health and care teams plan and deliver anticipatory care.

Anticipatory care is proactive, personalised health and care planning for people with complex needs.

Coordinated across the system, it can improve outcomes and help reduce demand for same-day appointments and services. 

Focussing on people with multiple long-term conditions, the BSW ICB toolkit is designed to help healthcare professionals identify patients who stand to benefit most from this approach and deliver care specific to their needs.  

This toolkit aims to support BSW ICB’s health and care teams in sharing best practice and increasing anticipatory care across the system.
