New courses support people living with long-term conditions

Patients in BSW living with long-term conditions are being given access to a new range of courses which have been developed by NHS South West, the South West Virtual Hub and Wiltshire Health and Care in conjunction with Hope For The Community CIC.

The Hope Programme is a free, online self-management programme for people living with any long-term condition to help them meet others in similar situations, feel more in control and more resilient.

The online courses can be accessed at any time and take about two hours to work through each week. They have been co-created by people living with long-term conditions and researchers at Coventry University.

Topics covered include:

  • Managing stress
  • Character strengths
  • Coping with fatigue
  • Smarter goal setting
  • Physical activity
  • Practising mindfulness
  • Gratitude
  • Eating well
  • Communication and relationships

The courses include worksheets, audio and video materials, quizzes, interactive activities and social networking via email and forums. Participants are supported throughout by trained facilitators with lived experience.

Upon completion of the course, participants can continue to have access to the online community, course content, activities and peer support.

The next course start dates are 18 July 2024 and 12 September 2024
