Wiltshire Council has a new 18-25 Moving On Service to help young people who have an eligible Care Act Need develop the skills for a rich, fulfilling and independent adult life.
Staff working in the 18 – 25 Moving On Service will develop the knowledge and expertise to support people to move from children’s services to adult services.
The new service went live on 1 August and the council is developing it over the coming months.
The aim is to start working with young people when they are 16, although responsibility will remain with children’s social care until they turn 18. The Moving On Service is on the same system as other adult services to make a smooth transition.
