BSW Together, the NHS integrated care system for Bath and North East Somerset, Swindon and Wiltshire is working towards improving how patients are discharged from hospitals.
Getting people back home from hospital, or to another setting such as a care home, safely and in a way that is planned with both the patient and their family and friend carers (if relevant) is a priority for our health and care services. We want to ensure that the voices of patients, carers and the public are at the heart of our work and influence the design and delivery of services.
BSW Together, which includes the acute hospitals in Salisbury, Bath and Swindon, wants to hear from people and their families who have recently experienced being discharged from a hospital.
We are looking for people to participate in an online survey, or in person or online focus group to discuss their experiences of hospital discharge and to help inform the development of resources that will help the discharge process.
